TORONTO, ONT (February, 2012) Canadian Cancer Society expands their successful SMAT program (Short Messages Against Tobacco) with groundbreaking personalized tools.

For the first time quitters can have real-time text chats with a live Quit Coach. Text and Chat Integrated (TaChI) is the brainchild of Evolution Health and the Quebec division of Canadian Cancer Society.

SMS-based quit programs are growing in popularity but are typically automated. The TaChI platform takes tailored SMS to a new level. In addition to receiving personalized, automated SMS messages, quitters can now chat directly with live Quit Coaches via their mobile phone. Coaches manage conversations via TaChI's web interface.

The tailored text service sends registrants daily scheduled motivational smoking cessation messages and replies to keyword prompts. In addition, quitters have the option of texting a live Quit Coach. The coach accesses member messages through the online TaChi interface. Sylvie Poissant, Director of Prevention with the Quebec division of Canadian Cancer Society noted, "TaChI is one of the first programs of its kind. We are excited to see how participants respond to the free service and to see how the SMAT and its personalized tools, including live chat by mobile phone, can impact quit rates."

According to Statistics Canada, 37,000 Canadians die every year due to smoking related illnesses. Most smokers attempt to quit multiple times before a successful quit. A failed or successful quit could mean the difference between life and death for many Canadians.

"Getting support whenever and wherever you need it from trained professionals could make the difference between a successful quit and another failed attempt" says Trevor van Mierlo, CEO of Evolution Health. "This program could revolutionize how health care systems provide immediate support."

The TaChI innovation is being piloted first in smoking cessation and will be introduced to new health behavior change support programs in the coming months. The addition of live texting with a Health Coach is an innovative and exciting new idea that has can potentially lead to numerous health behaviour change programs. "Evolution Health is known to provide top of the line programs, research-based innovations," says Trevor van Mierlo. "The TaChi service will potentially lead to numerous studies that can not only validate the program itself but also pave the way for more live health support programs via SMS." Campus Living Centres' "Check Your Drinking: Residence Life" provides college and university residence students with anonymous, non-judgmental feedback.

In August prior to the start of this academic year, Campus Living Centres, the largest owner and operator of student housing in Canada, launched a specialized online tool to help residence students make better decisions about their drinking.

Those 24 and under have nearly three times the rate of heavy frequent drinking compared to those 25 and over. Heavy drinking results in serious health consequences and increased emergency department admissions. However, there is strong evidence that certain interventions can help students stay healthy. Experts recommend interventions that combine cognitive-behavioral skills, normative comparisons and motivational feedback. Check Your Drinking based on these principles of Brief Intervention.

"Students spend a lot of their time online so the screener is an excellent way to reach those who may not have sought out information otherwise," says Tim Fricker, Director, Residence Life and the Student Experience at Campus Living Centres. "The instant feedback tells students the facts they need to know to make informed choices about their drinking."

Check Your Drinking Residence Life ( http://clc.checkyourdrinkingu.net ) is anonymous and takes less than 10 minutes to complete. Students receive a personalized Final Report that helps them understand their drinking patterns, and how their drinking affects their health, finances and studies. Randomized Controlled Trials have repeatedly shown Check Your Drinking helps problem drinkers significantly reduce their alcohol consumption.

"Check Your Drinking is an excellent and technically relevant way to educate students about the reality of alcohol consumption without lecturing or nagging, which we all know doesn't work," says Trevor van Mierlo, CEO of Evolution Health, the company behind Check Your Drinking. "Students overestimate the amount that their peers are drinking, so our technology helps correct these misconceptions. Plus, it can be really eye opening for students to see how much money they spend on alcohol, how many calories they consume, and relatively easy tips on how to reduce their risk."

Shepell•fgi wants you to go smoke free.

The company has launched a new online program to help employees kick the smoking habit. The program is an upgrade to smoking cessation services already offered under Shepell•fgi’s employee assistance program.
The new online component provides users with an interactive experience that includes tips, tools and a support network of health care professionals and peers.

Shepell•fgi’s ‘Stop Smoking Centre’ offers a nicotine dependency test and quit meter, which tracks the employee’s quit date, the number of smoke-free days he or she achieves and the amount of money saved by not buying cigarettes.
The program also includes access to an online support group, moderated by health care professionals.
“By delivering a robust program with more intuitive navigation and added features, we’re putting the user in control,” said Rita Fridella, senior vice-president and chief clinician, operations at Shepell•fgi.

“The tool broadens the continuum of ‘help options’ for our clients,” says Barb Veder, clinical director for Shepell•fgi. “Now you have a self-directed, interactive tool that supports employees whose lifestyle and learning style match this approach.”
Veder says that because the tool is customized, employees can jump in at any time, whether they’re contemplating quitting, attempting to quit, managing cravings or relapsing. “The intervention is not linear, but more dynamic, based on the employee’s needs at that stage,” she says.

“The program is customized to your unique requirements and preferences, making it easier for you to receive the right type and amount of support,” says Fridella.

More and more employers are offering smoking cessation programs as part of their benefits plans, and for good reason. Smoking can affect productivity—not only because a smoker’s schedule may be guided by his or her cravings, but also due to increased absences due to illness.

A 2006 study by the Conference Board of Canada  found that smokers take two more sick days per year than their non-smoking colleagues, translating—at the time—into an average cost of $323 per smoker per year in lost revenues to the employer.

Recently, the Toronto Star reported the Ontario government would begin funding smoking cessations drugs Champix and Zyban as part of the Ontario Drug Benefit Plan, effective August 4—a financial boon for employers currently offering coverage for those drugs under their benefits plans. Quebec, P.E.I. and Saskatchewan are currently the only other provinces to provide reimbursement for smoking cessation products. British Columbia is expected to follow suit in September.

What Makes Internet Weight Loss Programs Click? New Study Reveals User Motivation

Toronto / San Francisco, May 17, 2010: A study recently published by Evolution Health in the Journal of Medical Internet Research (JMIR) sheds new light on how people use Internet weight loss programs. In the study, the authors examined data from 204 users of The Healthy Weight Center who were clinically classified as either overweight or obese to see how they utilized the program. The purpose of the study was to examine the degree to which typical Internet site user accesses information and interactive exercises on nutrition, fitness and behavioral strategies, monitoring tools, as well as expert-moderated social networks. 

"This research is the first of its kind. The Healthy Weight Center is a free program that is not advertised, so the 204 users had to find the program through their own search efforts. They are true 'real-world' users and understanding their behavior is essential if we are to understand how the typical internet weight loss program user interfaces with online self-help and support communities" says Dr. Martin Binks, Clinical Director and CEO of Binks Behavioral Health and the study's lead author. "Most of what we know about online interventions to date is based on clinical trial data where participants are required to attend some in-person meetings and often have other restrictive selection criteria which is far from the typical online user's experience" says Binks.

The study found that the majority of registered members completed short exercises that gave personalized feedback and non-judgmental advice on their behavior. However, other tools that are highly promoted in other Internet weight loss programs, such as meal planners, nutrition lookup tools or activity logs were used less frequently. Although the study indicated that only a small percentage of users posted on the program's moderated social network, traffic patterns indicate high interest in reading peer discussions. 

"Our findings clearly support the fact that people living in the digital age want information that's fast and easily accessible, and we've seen this pattern evolve over the past decade," says Evolution Health Systems CEO and co-author Trevor van Mierlo. "The challenge Evolution Health faces when building eHealth programs or advising clients is to ensure that we capture and maintain the user's interest while providing enough information to change behavior. Data from all of our programs indicates that continuing to engage the user with brief cutting-edge exercises does increase adherence and retention, resulting in better outcomes for the insurance companies, large employers, pharmaceutical companies and governments that use our services.

The study concludes that while openly available, direct-to-the-public Internet weight loss programs can reach many consumers who cannot access traditional treatment, only certain types of content will keep these consumers engaged. "In other words, people in these types of widely available self-help programs appear not to be taking full advantage of traditional resources that would help them be successful over the long-term" says Binks, "so our challenge is to continually lead the area of Internet weight loss with motivational exercises and networking tools that will keep people clicking and coming back for more".

Unlike other online screening tools, About My Drinking immediately offers a variety of options for individuals to reduce their harm from alcohol, and the site provides information and guidance about other drugs including marijuana, cocaine, and prescription drugs. Participants also have the ability to speak 24/7 with a Hazelden counselor about a clinical assessment or treatment options.

While the site is designed to allow individuals to self assess their alcohol or other drug use, family members and other concerned persons may complete the screening on someone else's behalf. "We believe many people will use the site to seek support for a friend or loved one," said Carter.

Visitors to the site are able to:

  • Assess their current (or past) drinking patterns
  • Compare their drinking with others who are the same age and gender
  • Understand the difference between safe alcohol use, alcohol abuse, and alcohol dependence
  • Learn more about what they can do to help a friend or loved one
  • Learn about treatment and recovery options for problem use

University of Western Ontario students go online to check effects of drinking

Ask yourself - how much alcohol do I drink in a week or a year? What does it cost me to drink that much? What could I buy with that money? Does drinking affect my relationships and my health?

While these are important reflective questions for people of any age, they are questions that students at The University of Western Ontario are being encouraged to answer anonymously using a new on-line program that helps students assess the impact of their drinking.

The tool, called Check Your Drinking University (CYD-U), was developed by Evolution Health Systems, a company that develops and manages community-based interactive behavioural eHealth and disease management programs.

A version of the tool http://uwo.checkyourdrinkingu.net was developed especially for Western, by Evolution with support from John Cunningham, a senior researcher with the Centre for Addiction and Mental Health (CAMH). Cunningham's focus is addiction prevention.

A recent study by CAMH, and published in Addiction, shows that online tools such as these can help change drinking behaviours and Western officials are hoping it will be a good tool to promote responsible use of alcohol by students.

The tool calculates the annual amount spent on alcohol, the number of alcoholic drinks consumed per year, extra calories consumed from drinking and graphically illustrates the student's consumption patterns.

Jennifer Ashenden, Assistant Director of Residences at Western, says promoting responsible use of alcohol by students is a priority.

"We have education and awareness programs, alcohol policies, and we provide lots of opportunities for 'dry' events, including all Orientation Week activities," says Ashenden.

"We know the best steps are those that help students make the right choices themselves, and help them understand the implications of their choices. That's the goal of the Evolution tool."

To date at Western, the tool has mainly been used in an Alcohol Awareness Program designed for students who been identified as exhibiting drinking habits that could be problematic, but Ashenden says it will likely have broader use in the future.

Trevor van Mierlo, Evolution Health's Chief Executive Officer, is a Western alumnus (BA Hons, '96) who understands the importance of his company's tool.

"Like many other students, during my time at Western I was fortunate and learned to draw the line between having fun, focusing on my coursework and being responsible with my finances," says van Mierlo. "However, for many students there is tremendous peer pressure, and because there is a stigma associated with recognizing alcohol problems, many students are reluctant to voice their concerns. CYD-U is completely anonymous and accessible from any computer, so students can come to their own conclusions."

A CAMH Canadian Campus Survey conducted in 2004 found that 32 per cent of Canadian students displayed patterns of hazardous or harmful drinking (5+ drinks for men and 4+ drinks for women).

Approximately 95 per cent of Ontario university students reported drinking alcohol in the past year, and almost 30 per cent report drinking more than 15 drinks per week. It has been found that this type of harmful drinking is often accompanied by educational difficulties, psychosocial problems, antisocial behaviors, injuries, overdoses, high-risk sexual behaviors, and other risk taking, such as alcohol impaired driving.

Smokers' Helpline announces text messaging service

Hamilton, ON - October 20, 2009- The Canadian Cancer Society Smokers' Helpline has launched a text messaging cessation service element to assist tobacco users with quitting smoking. The service is available in English or French to Ontarians who can register at www.smokershelpline.ca. Standard text messaging rates apply but the service is otherwise free.

Smokers' Helpline Text Messaging (TXT) offers 11 to 13 weeks (depending on the registrant's quit date) of supportive messaging. Registrants can text back designated keywords that will generate additional, relevant messages at their identified time of need. TXT can be used alone or in combination with Smokers' Helpline phone service at 1 877 513-5333 and online program at smokershelpline.ca.

"A multi-modal approach to cessation intervention has been shown to increase one's chances of quitting smoking successfully," says Donna Czukar, Director, Cancer Information and Support at the Canadian Cancer Society. "Text messaging is a great way to support those who are trying to quit smoking when they are on the go and experiencing a craving. Text messaging is one more tool that Smokers' Helpline can offer people to help them quit smoking."

Smokers' Helpline Text Messaging is developed and managed by Evolution Health Systems, a leader in the growing field of e-Health programs and evaluation. "This service is the first of its kind in Canada," says Evolution's CEO, Trevor van Mierlo. "Smokers' Helpline has proven their commitment to innovation and we are thrilled to be involved with this project. There is very promising emerging evidence on the effectiveness of text messaging in behaviour change and we look forward to contributing to the science with further evaluation of the service."

The Canadian Cancer Society Smokers' Helpline is a free, confidential service that provides personalized support, advice and information about quitting smoking and tobacco use. SunLife Healthy Weight Centre

Evolution Health and SunLife Financial have partnered to develop a customized version of the Healthy Weight Center(HWC). Launched in April, 2009, the vibrant weight management tool features an expert moderated support group, text messaging, motivational emails, goal setting exercises as well as an "Ask the Expert" feature. Within the program, members can engage in dynamic education modules, use the HWC meal plans, find nutritional information through the Nutrition Database and reassess their BMI. Members also have access to a dynamic journal to help them track their daily calorie intake and expenditure, their mood, and weight in addition to a food log.

Check Your Drinking (CYD) Brazil

Evolution Health and the Social Psychology and Collective Health Research Group (POPSS) from the Department of Psychology of the Federal University of Juiz de Fora (UFJF) have partnered in the creation of an adapted version of the Check Your Drinking (CYD) Screener for Brazil. The POPSS's commitment to excellence has been demonstrated by their ongoing dedication to research and interventions for alcohol and drug abuse in Brazil. The CYD screener is an 18 question survey that is anonymous, and is designed to provide the user with normative feedback regarding their drinking. The CYD screener is the first of its kind in Brazil. We have a Winner!

We are happy to announce that Rylee Richards of Thunder Bay was the grand prize winner of the 2009 Driven to Quit Challenge. Rylee was the lucky recipient of a brand new 2009 Chevrolet Malibu hybrid! Two secondary prize winners were awarded a $5,000 vacation, and seven winners were awarded a $2,000 Canadian Tire gift card. Each of the 10 winners' "support buddies" also received a $250 Canadian Tire gift card. The winners were randomly drawn from the 22, 365 Ontarians who participated in the Driven to Quit Challenge this year, and prizes were awarded at a formal ceremony in Toronto. Since 2006, the Driven to Quit Challenge has inspired more than 101,000 Ontarians to make a quit attempt.

First Foreign Company to Receive Prestigious JIMA Trust Mark

We are proud to announce that Evolution Health Systems Inc. is the first non-Japanese company to be awarded the prestigious Japanese Internet Medical Association (JIMA) Trust Mark for the Web-Based Depression and Anxiety Test (WB-DAT). The WB-DAT is a peer-reviewed, online computer program that anonymously detects different types of depression through the administration of a short test. At the end of the test, users are provided with a final report that contains the results of their test and lists their symptoms of depression. The final report is designed to be shared with a doctor, counsellor, or other health care professional. The WB-DAT is an effective tool that anonymously reaches those who are depressed, and is expected to raise awareness about depression in Japan. According to Hiroaki Mitani, the Secretary General of JIMA , "On the Japanese Internet, the WB-DAT is unique and is the first confidential tool that can motivate citizens to seek professional help. The WB-DAT can also assist experts in assessing patients". Japanese-language versions of the WB-DAT can be accessed free-of-charge at the Tokyo English Life Line website (http://telljp.wb-dat.net) and (http://www.wb-dat.net).

First Foreign Company to Receive Prestigious JIMA Trust Mark

Evolution Health Systems Inc. is the first non-Japanese company to be awarded the prestigious Japanese Internet Medical Association (JIMA) Trust Mark (http://www.jima.or.jp) for the company's Web-Based Depression and Anxiety Test (WB-DAT).

"We awarded the JIMA Trust Mark to Evolution Health's Japanese-language WB-DAT as it passed our rigorous eHealth code of ethics", says Hiroaki Mitani, the secretary general of JIMA. "This is the first time for us to award the JIMA Trust Mark to a non-Japanese based Internet program and we expect the WB-DAT to raise awareness about depression in Japan." The WB-DAT is a peer-reviewed online computer program that anonymously detects different types of depression. Japanese-language versions of the WB-DAT can be accessed free-of-charge at the Tokyo English Life Line website (http://telljp.wb-dat.net) and (http://www.wb-dat.net).

At the end of the test users are provided with a final report that lists any symptoms of depression. The final report is designed to be shared with a doctor, counselor or other health care professional.

"The number of the patients with depression in Japan has been increasing, and we feel that the WB-DAT is an effective tool that anonymously reaches those who are depressed," says Mitani-san. "On the Japanese Internet the WB-DAT is unique, and is the first confidential tool that can motivate citizens to seek professional help. The WB-DAT can also assist experts in assessing patients."

"eHealth research in Japan is exciting, however language and cultural differences can pose significant roadblocks," says Dr. Peter Farvolden, Evolution Health's Clinical Advisor for Mental Health. "The JIMA Trust Mark is a very important accreditation that has already helped us gain acceptance from patients, the Japanese scientific community and our research partners."

According to the World Health Organization, depression is the leading cause of disability among men and women of all ages. In addition to the devastating personal consequences, depression is very costly to employers as employees suffering from depression miss an average of 30 to 50 workdays per year. It is estimated that about half of all people suffering from depression remain undiagnosed.

TELL partners with Evolution Health Systems to encourage citizens to seek help for depressive symptoms

Tokyo, Japan / Toronto, Canada: Tuesday, December 2, 2008

In an effort to encourage those who may be affected by depression to seek help, TELL, a provider of free, confidential telephone counseling and support , as well as face to face counseling in the Kanto region, today launched their version of the Web-Based Depression and Anxiety Test (WB-DAT) at http://telljp.wb-dat.net.

Launched in English, WB-DAT will soon be available in Japanese as well.

"Although our telephone counseling service is available 365 days a year, many people who can benefit from our services may be too shy or nervous to pick up the phone", says Jason Chare, TELL's Director of Life Line Services. "As the WB-DAT is online and accessible 24 hours a day, 7 days a week, it may give some people a starting point and encourage them to call the Life Line  or seek assistance from a counselor or a doctor."

The WB-DAT, a peer-reviewed online computer program, anonymously assesses users for different types of depression. "Internet-based or electronic assessments can never replace a doctor's diagnosis" says Dr. Peter Farvolden, Evolution Health's Clinical Advisor for Mental Health. Check Your Drinking University (CYD U)

Check Your Drinking University (CYD U) is the latest addition to Evolution Health suite of eHealth Brief Interventions. The screener is modeled after the second version of Check Your Drinking while specifically targeting college and university students. The screener is designed to help students understand how much they drink compared to other students, how much money they spend on alcohol or what their personal risks could be.

Canadian Action Network for the Advancement, Dissemination and Adoption of Practice-informed Tobacco Treatment (CAN-ADAPTT)

Evolution Health and Centre for Addiction and Mental Health (CAMH) have collaborated again to create a new interactive and evolving website available to all professionals in the field of tobacco cessation. The Canadian Action Network for the Advancement, Dissemination and Adoption of Practice-informed Tobacco Treatment (CAN-ADAPTT) is a Practice-Based Research Network (PBRN) committed to facilitating research and knowledge exchange between practitioners, healthcare/service providers, etc. and researchers in the area of smoking cessation.

Features of the site include a wiki application where Clinical Practice Guidelines can be frequently updated and adapted to reach all populations and a discussion group where members can discuss and debate the best practice.

Smokers Helpline Online

After much anticipation, Smokers’ Helpline Online has migrated to the Stop Smoking Centre version 6.0 platform and expanded its network to five additional provinces. With a new look and feel the upgrade focused on building community and highlighting members success stories by adding to its already existing Hall of Fame and Anniversaries pages and creating a new Testimonial page. The new ‘Quit Centre’ embraces the open concept of the updated version allowing members to freely move through the program, choosing exercises as they see fit.

When asked how such a website could help people who might need therapy the most, yet live without access to a computer or Internet, University of Saskatchewan psychiatry professor Carl D'Arcy said the more people can help themselves, the more health professionals will be free to counsel those patients without web access. Evolution Health Systems releases AlcoholHelpCenter 2.0 with enhanced Social Networking and Structured Relapse Prevention

According to national surveys conducted by the CDC, over half of the adult US population drank alcohol in the past 30 days. Approximately, 5% of the total population drank heavily while 15% of the population binge drank.

Evolution Health Systems Inc. today launched an enhanced version of its eHealth program for problem drinking: Alcohol Help Center 2.0 (AHC 2.0). The new program can be freely accessed at http://www.alcoholhelpcenter.net.

“Among problem drinkers, Internet access is surprisingly high, with 81% of Ontario adult problem drinkers reporting access to the Internet from their home,” says Dr. John Cunningham, Evolution Health’s Clinical Advisor for Alcohol and Substance Abuse. “The program’s evidence-based tools have been used in paper format for quite some time, however online tools offer tailored feedback quickly and anonymously.”

AHC 2.0 offers users a number of electronic exercises that have been designed to help people cut down or quit drinking. The new program includes self-guided Structured Relapse Prevention (SRP) modules, anonymous blogs, email and text message tips, and additions to the program’s expert moderated online social network. Like the previous version, Alcohol Help Center 2.0 is anonymous and is accessible through the Internet 24 hours a day, seven days a week.  

“With the enhancements to AHC 2.0, program members will have more coping methods and trouble shooting strategies to share and discuss in the online support groups“, says Trevor van Mierlo, the company’s Chief Executive Officer. “It’s certainly an exciting time for Internet-based brief interventions, behavior modification programs and healthcare based online social networks.”   

Hazardous alcohol consumption has been identified as one of the five leading contributors to the global burden of disease and results in significant economic cost. Unfortunately, mainly because of social stigmas, many problem drinkers do not seek treatment.

“Alcohol problems are costly to employers, insurers and cause tremendous economic burden”, says Rachel Fournier, Director of Business Development. “People with drinking problems say they call in sick or skip work 30% more than workers who don’t have drinking problems, and are also more likely to be late for work or leave early.”

With 65% of the American adult population being overweight or obese and 17% of children and adolescents falling into the overweight category, obesity rates have reached epidemic proportions. Evolution Health Systems Inc., the leading provider of electronic health programs and services, has officially launched the sixth version of StopSmokingCenter.net, the highly tailored Web-Assisted Tobacco Intervention (WATI).

The new eHealth program (SSC 6) blends tailoring, personalized feedback, dynamically generated cessation exercises, progress tracking, text messaging, and custom email programs with online social networking and instant messaging. Clients who license customized versions of the technology are also provided access to a web-based data reporting interface with real-time usability and Return on Investment data.

“We kicked-off the redevelopment cycle by consulting all of our key stakeholders including longstanding members of SSC version 5, usability experts, our Clinical Advisors, the Health Educators who manage our Online Social Networks, our Application Architects, Program Developers, Project Managers, and the most connected and technologically sophisticated members of our staff: our junior employees,” says Trevor van Mierlo, Evolution Health’s CEO. “The result is truly a collaborative platform that Evolution Health will extend its other eHealth platforms and technologies.”

“In developing SSC 6 we engineered digital algorithms that filtered smokers into tailored programs,” says Dr. Peter Selby, Evolution Health’s Clinical Advisor for Addictions. “As quitters complete dynamic exercises at their own pace, they’re encouraged to anonymously network with others program members, so peer-support and encouragement is available 24 hours a day, seven days a week – or when smoker’s need it most.”

Before its official release, the beta version of the SSC 6 had undergone extensive usability testing at the Centre for Global eHealth Innovation in Toronto. “Following a heuristic evaluation, usability changes were made to the core structure of the program,” says Deborah Chan, Human Factors Engineer, Healthcare Human Factors Group. “Our next step was to recruit smokers to test the key functionality and provide feedback about usability using our think aloud protocol. The recommendations resulting form the testing helped organize SSC 6 in a more meaningful and intuitive way, increasing the reach and impact of the intervention.”

In a lecture presented at the 2007 Society for the Study of Addiction (SSA) Annual Symposium in York, UK, researchers found that 25% of SSC members who made their first post in the program’s social network received a response within 12 minutes. “In an offline setting, it would be cost prohibitive to offer this kind of rapid support from a professional,” says Dr. John Cunningham, Evolution Health’s Clinical Advisor for Substance Abuse and a co-author of the study. “We have much to learn from online social networking but the results from this and other studies are encouraging.”

“We’re thrilled about the success of the new program” says Rachel Fournier, Director of Business Development. “Our member and customer feedback has been instrumental in the redesign. It’s been a two year development cycle, but we’ve been listening to our client’s needs and have developed a program that gives quitters simple access to amazing tools to engage and connect with other quitters.”

Abstract

Objective: Social support networks have been identified as one factor that can help people resolve their problem drinking. A relatively new phenomenon is online support groups, such as the one provided on the Alcohol Help Center (AHC: http://www.alcoholhelpcenter.net). Preliminary use of the AHC support group will be summarized and lessons learned in the initial development of an online support group will be discussed.

Methods: The AHC support group can be viewed by anyone interested in its content. However, only registered users of the AHC can make postings and trained professional staff moderates all support group content. The amount and content of traffic on this support group were recorded. Qualitative analyses were conducted to identify the types of message content.

Results: There were 674 posts on the AHC during the first 10 months of operation, including those made by the professional moderators. Content analyses of these postings identified common themes, including introductions, greetings, general supportive statements, suggested strategies, success stories, and discussion of difficulties. In addition, it appeared that the amount of traffic on the support group varied over time and clustered around nodes that consisted of one or more active users. This variation in traffic may disappear over time as overall use of the AHC support group increases.

Conclusions: Online support groups are developing into a new venue of support for those with drinking problems and other addictions concerns.

Practice implications: Consider referring clients to an online, professionally moderated, support group as another means to help problem drinkers.

The many social, mental and economic costs related to problem gambling are not well known. Approximately 5% of the adult Canadian population (1.2 million) have exhibited behaviour that would classify them as at-risk or problem gamblers. In the United States, the estimated economic cost of problem gambling is $5 billion.

Abstract: The functional overlap of smoking and problem drinking has led researchers to speculate on the need for integrated treatment models. What of online services? With the burgeoning growth of Web-based interventions for smokers and the increasingly common online services for problem drinkers, there is the potential to provide options for smokers to also deal with any alcohol concerns.

The integration of these services might also help increase smoking cessation rates, because alcohol consumption is a known trigger to smoking and also for relapse to smoking. This paper presents results of the use of an online personalized feedback assessment for drinking (Check Your Drinking, CYD) by smokers who were recruited from the Stop Smoking Center (SSC; http://www.stopsmokingcenter.net. Registered users of the SSC (N = 7,741) were invited to complete the CYD (now located as part of an online alcohol reduction program freely available at www.alcoholhelpcenter.net). A total of 963 SSC users responded to the invitation, providing information about their drinking as well as a summary of their current smoking and past experiences of alcohol functioning as a trigger for smoking.

One-third of current daily smokers were problem drinkers (24% of occasional smokers and 22% of former smokers were current problem drinkers). Most (82%) daily smokers who were current drinkers reported they frequently or always experienced a strong urge, desire or thoughts about smoking when they drank alcohol.

This brief report will explore the implications of the overlap of smoking and drinking by these online participants and will discuss the potential benefits of providing an integrated service for smokers and problem drinkers.

Fédération Québécoise des Centres de Réadaptation pour Personnes Alcooliques et autres Toxicomanes (FQCRPAT) and Evolution Health Systems Inc. (www.evolutionhs.com) today announced the launch of a customized version of Evolution Health’s Web-Based Depression and Anxiety Test (WB-DAT). The FQCRPAT version of the WB-DAT is anonymous and freely available at: http://wb-dat.fqcrpat.org/.

According to the World Health Organization five of the 10 leading causes of disability are related to mental disorders. Recent estimates put the lifetime prevalence of mood disorders to be 20.8% and anxiety disorders at 28.8%. Although mood and anxiety disorders are imminently treatable, early detection still proves to be a major barrier. For example, only about 50% of those suffering from Depression are diagnosed.

The WB-DAT screens for the depressive, anxiety and impulse control disorders. Research shows the screener is sensitive and specific; however the smart skip-logic means a thorough assessment usually takes no more than seven to ten minutes. Patients are presented with a detailed, personalized Final Report that summarizes their responses to major diagnostic criteria. People who are concerned that they may be suffering from a mood or anxiety disorder are encouraged to take their Final Report to their family doctor, or email their results directly to their mental health professional.

The WB-DAT is the first peer-reviewed and clinically validated screener for mood and anxiety disorders. "A Web-Based Screening Instrument for Depression and Anxiety Disorders in Primary Care", a peer reviewed study on the WB-DAT, was published in the Journal of Medial Internet Research: www.jmir.org/2003/3/e23/.

"Despite the growing awareness of mental health disorders, under diagnosis remains a major barrier to treatment. Fortunately, online services can help both by offering an anonymous, discreet assessment for those who are unsure how to seek help in person, and by reaching those who live outside of major metropolitan areas lacking treatment centres," says Dr. Peter Farvolden, Evolution Health's Clinical Advisor for Mental Health. "Our relationship with the FQCRPAT will ultimately allow us to understand how anonymous electronic assessments can reach a wide variety of geographic and culturally diverse patients."

FQCRPAT’s customized version of the WB-DAT will help the FQCRPAT to reach Quebec residents who may not be able otherwise to access this kind of support service. "The FQCRPAT website has the potential to assist thousands of Quebec residents who would otherwise have limited access to mental health screening services,” says Dr. David Ross, Ph.D., Chair of the FQCRPAT Concurrent Disorders Committee. "In a province as large as Quebec we identified the need to provide personalized, online support that could reach socially and/or geographically isolated.

Evolution Health and FQCRPAT will be analyzing anonymous data sets generated from the WB-DAT to gain further understanding of how Web-based diagnostic tools and support services can decrease or offset the traditional costs associated with treating and managing mental health issues. Specific strategies include examining the geographic locations of WB-DAT users, to determine whether the program is successful in reaching individuals who live in rural areas and have limited access to mental health services.

Evolution Health Announces Peer-Reviewed Study of the Check Your Drinking Screener

"Formulative Evaluation and Three-Month Follow-Up of an Online Personalized Assessment Feedback Intervention for Problem Drinkers" has been published and can be accessed from the Research & Development section of the Evolution Health Corporate Website.

Background: In recent years, online services for problem drinkers have been developed. This paper describes ongoing efforts to improve one of these services, the Alcohol Help Center.

Objective: This report summarizes new modules added to the Check Your Drinking (CYD) screener, a component of the Alcohol Help Center, to make the CYD screener more useful to periodic heavy drinkers, as well as to regular alcohol consumers. Participants’ initial reactions to the CYD screener and the changes in their drinking habits at a three-month follow-up are presented.

Methods: The CYD screener provides a free personalized Final Report that compares the user’s drinking to that of others in the general population of the same age, gender, and country of origin. Current alcohol consumption and demographic characteristics are collected as part of the CYD screening process. After users were presented with a customized Final Report, they were hot-linked to a volunteer feedback survey. The voluntary feedback survey asked about impressions of the CYD Final Report. Respondents agreeing to participate were sent a follow-up survey after three months.

Results: We recruited 388 volunteers (69% female) who were registered users of another free-to-consumer online eHealth service. Of the 343 respondents agreeing to participate in the three-month follow-up, 138 accessed the survey, and 97 provided complete data (participation rate = 40%; completion rate = 70%). Compared to moderate drinkers, current problem drinkers judged the Final Report to be more useful (34% vs. 69%, χ21 = 41.5, P < .001) and accurate (43% vs. 76%, χ21 = 36.0, P < .001). Respondents who participated in the three-month follow-up displayed reductions in drinking compared to baseline (F4,76 = 12.2, P = .001).

Major Depressive Disorder is the most common psychiatric illness with a lifetime prevalence of 16.6%.

Evolution Health Systems Inc. today announced the launch of Check Your Drinking (CYD), an anonymous electronic assessment that provides personalized and comparative feedback on alcohol consumption and consequences. The CYD live demo is now freely available http://www.checkyourdrinking.net/.

Approximately one of every 13 adults has a serious problem with alcohol, yet only 2-3 million seek help each year. Left undiagnosed and untreated, alcohol problems result in staggering consequences to individuals, health care systems and employers. For example:

  • Almost half of all trauma and injury visits to hospital emergency rooms and 20-30% of all trips to the emergency room are alcohol-related.
  • People with drinking problems say they call in sick or skip work 30% more than workers who don't have drinking problems. They also are more likely to be late for work or to leave early.

CYD is a brief, web-based screener that compares drinking patterns to others of the same nationality, sex and age. Along with identifying health or career risks, CYD also calculates the annual amount spent on alcohol, the number of alcoholic drinks consumed, extra calories consumed and assesses alcohol consumption patterns.

The discreet, anonymous nature of electronic assessments may offer an important means of reaching those suffering from undiagnosed addictive behaviors. As a result, some states and provinces are beginning to mandate electronic screening and electronic tools as a cost-effective means to deliver standardized assessments on the population level.

"The CYD assessment provides all of the information that a health care professional needs to identify people whose alcohol consumption has become hazardous or harmful to their health,” says Dr. John Cunningham, Evolution Health's Clinical Advisor for Alcohol and Substance Abuse and research scientist at the Centre for Addiction and Mental Health. "These evidence-based tools have used in paper format for some time, but can be cumbersome to score and costly to distribute. Plus, there is a tremendous stigma with alcohol use, so many are reluctant to discuss concerns. The electronic CYD expands reach to an anonymous, highly sensitive, personalized assessment. We hope that providing this tool to health care professionals will allow them to help more patients.”

The WB-DAT, peer reviewed in the Journal for Medical Internet Research (www.jmir.org/2003/3/e23/), is a brief, web-based screener that is reliable for identifying patients who suffer from depressive and anxiety disorders.

Depressive disorders affect an estimated 9.5 percent of adult Americans in a given year, or about 19 million people. They are the leading cause of disability among men and women of all ages in the U.S. and worldwide, yet it is estimated that about half of all people suffering from Depression remained undiagnosed.

Anxiety disorders are the most common psychiatric illnesses. Like depressive disorders, anxiety disorders are highly treatable, yet only about one-third of those suffering from an anxiety disorder receive treatment.

The discreet, anonymous nature of online assessments may offer an important means of reaching those suffering from undiagnosed depressive and anxiety disorders. Some states are even beginning to mandate mental health screening and electronic tools are viewed as a cost-effective means to deliver standardized assessments on the population level.

Before the launch of http://www.wb-dat.net/ customized versions of the application were embedded in Evolution Health behavior modification and Cognitive Behavioral Therapy (CBT) programs. Over 115,000 people have completed the embedded versions of the WB-DAT since first being launched in March 2002, and 36% of those users surveyed have indicated that they will show their Final Reports to their health care professional.

With an overburdened healthcare system physicians simply do not have the time it takes to thoroughly screen for MDD and anxiety disorders, says Dr. Peter Farvolden, Evolution Healths Clinical Consultant for Mental Health. The WB-DAT allows people to complete a highly sensitive, easy to use yet specific algorithm. It takes the average patient 10 minutes to complete the test and when they are finished they can give their Final Report to their healthcare professional. We believe that the test will allow healthcare professionals to help more patients.

Due to the tremendous reach and effectiveness of the WB-DAT we decided to provide stand-alone versions in multiple languages for the public, says Trevor van Mierlo, Evolution Healths CEO. Reducing the Burden of Panic Symptoms: Evolution Health Announces Peer-Reviewed Study of the Panic Program

(Evolution Health) today announced the publication of “Usage and Longitudinal Effectiveness of a Web-Based Self-Help Cognitive Behavioral Therapy Program for Panic Disorder“ in the Journal of Medical Internet Research (JMIR). The peer-reviewed study analyzes results from the Panic Program, Evolution Health’s freely available Web-based Cognitive Behavioral Therapy (CBT) program. A live demo of the 12-session, self-guided program can be found at http://www.paniccenter.net/.

The peer-review study outlines the tremendous and positive impact of the Panic Program. As might be expected, because of it’s world-wide availability and no-fee structure, use of the Panic Program was associated with the attrition rates observed for other freely accessible self-help materials. However, the “real-world” data remain compelling.

Users reported statistically significant reductions in panic attack frequency and attack severity (P<.002). Of the 152 users who completed only 3 sessions of the 12 session program, the average number of attacks per day were reduced by 39% (P<.001).

“The data we collected and continue to collect is very compelling,” says Dr. Peter Farvolden, Evolution Health’s Clinical Consultant for Mental Health. “We know that CBT for panic disorder is a very effective treatment. It’s clear that access to the Panic Program contributes to the decrease of panic symptoms, and as the majority of panic suffers reported a decrease in their symptoms early on in the program, a few sessions of the Panic Program may be all that some require. In addition, the more people use the program, the more they seem to benefit. People who stick with the program through all 12 sessions virtually eliminate their panic symptoms.”

Anxiety disorders are common problems that result in enormous suffering and economic costs. Unfortunately, a large proportion of people who suffer from anxiety disorder remain either untreated or inadequately treated.

In addition to personal suffering and disability, anxiety disorders impose a significant economic burden on the healthcare system. According to recent research anxiety disorders cost the U.S. at least $42 billion a year. $22.84 billion of the $42 billion is associated with the repeated use of the healthcare system by people seeking relief from anxiety symptoms. Although effective treatments for panic disorder are available, access to treatment is limited.

Web-based self-help can offer very effective treatment insofar as it has the potential to be interactive, tailored to an individual’s specific needs, able to monitor progress and offer peer support, and augment the traditional physician-patient.

MDAO Website Expands Ability to Help Others with the Launch of Evolution Health Web-Based Diagnostic Tools and Online Support Services

The enhanced sites allow MDAO to expand its services to be able to reach more people who may not be able to access in-person support. Site visitors now have 24/7 access to interactive and community-building tools via Evolution Health's Web-Based Depression and Anxiety Test (WB-DAT) and WebTriage Online Support Group moderation software.

The WB-DAT is the first peer-reviewed and clinically validated screener and has US and Canadian Patents Pending. Published results are available under "A Web-Based Screening Instrument for Depression and Anxiety Disorders in Primary Care" in the Journal of Medial Internet Research: www.jmir.org/2003/3/e23/.

In North America general practitioners (GPs) tend to be the gatekeepers of mental health care. An estimated 5-50% of doctor visits are by patients who suffer from MDD and anxiety disorders. However, current data suggests that GPs fail to diagnose up to 50% of patients because those seeking assistance often consult with their GPs for other physical complaints. The WB-DAT screener increases awareness and assists with the accurate diagnosis of mood and anxiety disorders. The Final Report summarizes a patient's responses to major diagnostic criteria. People who are concerned that they may be suffering from a mood or anxiety disorder are encouraged to take their algorithm-generated Final Report to their family physician, or email their results directly to their mental health professional.

MDAO's new family of online support groups, moderated by MDAO's staff of volunteers, is based on a customized version of Evolution Health's WebTraige patient support software. The web-based communities are a natural extension of MDAO's extensive network of in-person support groups. WebTriage allows moderators to proactively manage support group activity, respond to needs of anonymous users who are seeking advice, while facilitating and promoting peer-based support. The software also produces summary reports that tracks support group usage and moderator tasks.

"Early on we realized that the Internet and the MDAO website had the potential to assist thousands of Ontarians who required actionable information and support," says Karen Lieberman, MDAO's Executive Director. "Our in-person Support Groups have very positive results; however with a province as large as Ontario we identified the need to provide similar online support services that can serve as a single meeting place for geographically and socially isolated individuals who might not otherwise have access to peer support services."

Panic disorder is an anxiety disorder that affects 2.4 million Americans each year. Women are twice as likely to be affected as men, and less than 25% of North Americans with an anxiety disorder receive treatment.

Van Mierlo Communications Consulting Inc. (Evolution Health) today announced the release of Stop Smoking Center (SSC) Support Group Software version 4.0. Version 4.0 contains several upgrades, including a new user interface, enhanced patient tracking capabilities and a new Patient Communication Interface (PCI). "Our new Support Group offers several key features to motivate quitters, improve compliance and prevent relapse," says Melanie Pereira, RN, Site Director for the SSC. The decision to launch the booklet was based on a large volume of requests from primary care physicians, smoking cessation specialists, pharmacists and smokers who use the online version of the program.

"Despite the fact that Internet usage continues to rise in both homes and workplace, there are thousands of people using our program who do not have 24-hour access," says Melanie Pereira (RN), Site Director for the Stop Smoking Center. "We have been inundated with requests from doctors and pharmacists who ask for Stop Smoking Center materials to give to their patients. Making the online program available in booklet format will allow our members to keep smoking cessation top-of-mind while away from the computer."

Worldwide adoption of the Internet has enabled smokers to anonymously access online smoking cessation programs. At the end of 2003, 68% of American adults and 71% of Canadian adults were online.

Evolution Health Systems Inc. (Evolution Health) today announced the launch of the Nicotine Dependence Test, an electronic assessment that analyzes a smoker's level of nicotine dependence, nicotine consumption rate and readiness to quit.

The software application, available at the Stop Smoking Center (http://www.stopsmokingcenter.net/), provides a printer-friendly Final Report that smokers can use when consulting with a primary care practitioner or pharmacist. Smokers can also email the Final Report to their health care professional.

"Research indicates that smokers can significantly increase their chances of quitting if they are well prepared," says Trevor van Mierlo, Evolution Health's CEO. "The Nicotine Dependence Test was designed to educate smokers on the particulars of their habit, which is the first step in creating a personalized plan that will help in the transition to a smoke-free lifestyle."

Evolution Health made the decision to invest in the technology based on the success of the Depression Test (www.DepressionCenter.net/DepressionTest). In October of 2003 a validation study of the test was published in the Journal of Medical Internet Research (http://www.jmir.org/), and since publication an increasing number of primary care physicians and public hospitals have utilized the test as a screening mechanism for new patients effected with either depression or anxiety.

TORONTO, Oct. 1 /PRNewswire/ -- Evolution Health Systems Inc. (Evolution Health) today announced the publication of WB-DAT: a Web-Based Screening Instrument for Depression and Anxiety Disorders in Primary Care in the September issue of the Journal of Medical Internet Research (www.jmir.org).

The study outlines preliminary validation data for 193 patients who were being assessed and/or treated at the Mood and Anxiety Program and Clinical Research Department at the Centre for Addiction and Mental Health (www.CAMH.net) in Toronto Canada. The patients completed the Depression Test before being interviewed with the Structured Clinical Interview for DSM-IV Axis I Disorders (SCID-I/P). Diagnostic data from the Depression Test was then compared to data from the patients interview.

The Depression Test (patent pending) is a freely available web-based screener located online at www.DepressionCenter.net/DepressionTest. The test is designed to screen for Major Depressive Disorder (MDD) and anxiety disorders. It produces an algorithm-generated Final Report that summarizes a patients responses to major diagnostic criteria. Users are encouraged to print their Final Report and bring it to their family physician, or email the results directly to their healthcare provider. The test is also available at The Panic Center where questions relating to anxiety disorders appear first (www.PanicCenter.net/AnxietyTest)

In North America general practitioners (GPs) tend to be the gatekeepers of mental health care. An estimated 5-50% of doctor visits are by patients who suffer from MDD and anxiety disorders. However, current data suggests that GPs fail to diagnose up to 50% of patients because those seeking assistance often consult with their GPs for other physical complaints.

Panic disorder is an anxiety disorder that affects 2.4 million Americans each year. Women are twice as likely to be affected as men, and less than 25% of North Americans with an anxiety disorder receive treatment.

With an overburdened healthcare system physicians simply do not have the time it takes to thoroughly screen for MDD and anxiety disorders, says Dr. Peter Farvolden, the first author of the study. The Depression Test allows people to complete a highly sensitive, easy to use yet specific algorithm. It takes the average patient 10 minutes to complete the test and when they are finished they can give their Final Report to their healthcare professional. We believe that the test will allow healthcare professionals to help more patients.

In addition to personal suffering and disability, anxiety disorders impose a significant economic burden on the healthcare system. According to recent research anxiety disorders cost the U.S. at least $42 billion a year, almost one third of total mental health expenditures ($148 billion). $22.84 billion of the $42 billion is associated with the repeated use of the healthcare system by people seeking relief from anxiety symptoms. Although effective treatments for panic disorder are available, access to treatment is limited.

The Depression Test is a valuable resource for patients, physicians and the entire healthcare spectrum, says Trevor van Mierlo, Managing Director for Evolution Health. TORONTO, June 16 /CNW/ - An online web-assisted support group for people dealing with the stress of SARS, http://www.sarssupportcentre.net/, was launched today by the Centre for Addiction and Mental Health (CAMH).

"SARS has been stressful both for people who have been personally affected by it and for members of the public who have questions and concerns about the illness," said Dr. Peter Selby, Head of CAMH's Nicotine Dependence Clinic and Assistant Professor, Departments of Family and Community Medicine, Public Health Sciences and Psychiatry, University of Toronto. Selby came up with the idea for the online support when two of his medical colleagues from other health care centres developed SARS.

"At a time when there is restricted access to the health care system, this medium allows for the delivery of service to a segment of the population with access to the Internet, including health care workers, at a very reasonable cost to the system. The technology allows for the delivery of sophisticated information and self-help tools as well as links to the latest information on SARS," said Dr. Peter Farvolden, a research scientist in CAMH's Clinical Research Department. Farvolden, along with Dr. David Goldbloom, CAMH Physician-in-Chief, and David Marsh, Clinical Director, Addiction Medicine, are involved with the development of the site.

Selby envisions the web site as being used by people who are in quarantine; patients in isolation who have SARS but who are well enough to use a computer and who have little support because contact is limited due to restrictions; and members of the public who are feeling stressed by the SARS situation and who want to educate themselves on ways to cope with the stress. It was estimated in April 2003 that there were over 6 million internet searches for SARS information worldwide.

The site features a section for health care providers and another for the lay public with an online support group moderated by a trained health care professional. The support group is anonymous and free for anyone to use. Initially, it will be offered in English only, but translation will be explored.

CAMH is sponsoring the site and has contracted with Evolution Health Systems Inc., a Toronto based software company experienced in online collaborative self-help, to host the site. "One of the most important aspects of our program is that we show people they are not alone," said Trevor van Mierlo, Evolution Health's CEO. "And when provided with the right tools and information, our visitors feel they have greater control."

The web site will be evaluated to determine the effectiveness of the medium as a means to respond to other acute stressors, disasters and/or epidemics, such as bioterrorism, and West-Nile virus.

The Centre for Addiction and Mental Health is a Pan American Health Organization and World Health Organization Collaborating Centre and a teaching hospital fully affiliated with the University of Toronto.

The Depression Center provides educational content for physicians and patients, a glossary of medical terms, a glossary of terms specific to Depression, and features the Depression Test (patent pending), a brief screener based on existing diagnostic criteria.

The Depression Center also offers an expert moderated Online Support Group, where members can share stories, discuss treatment options, and get advice. The program is free for both patients and doctors.

New Website to Help Pregnant and Postpartum Women Stop Smoking

Up to one-quarter of pregnant women in Ontario smoke. The first website providing support for these women and those who are postpartum and their health providers was launched today by PREGNETS, a coalition of groups dedicated to preventing gestational and neonatal exposure to tobacco smoke. The website can be found at http://www.pregnets.org/.

"Smoking during pregnancy has serious harmful effects on the woman, the fetus and the child. Unfortunately, pregnant women don’t often get the information that they need to help them to stop. Sometimes mothers do not want to talk to their health providers about their smoking due to the stigma. This website will assist women to access smoking cessation resources in their community and provide confidential assistance," says Peter Selby, the project leader and head of the Centre for Addiction and Mental Health’s Nicotine Dependence Clinic and Assistant Professor, Departments of Family and Community Medicine and Psychiatry, University of Toronto.

The effects of smoking during pregnancy include an increased risk of spontaneous abortion, lower birth weight and difficulty feeding. Children who are exposed to second hand smoke have an increased risk of having SIDS and of developing asthma, allergies, bronchitis and learning disabilities.

In addition to the website, over the next two years, PREGNETS will train existing smoking cessation resource centres including the Smoker’s Helpline and Motherisk at the Hospital for Sick Children to provide assistance to pregnant and postpartum mothers. A one-page reference tool will also be developed for health care providers, including family physicians, outlining the harmful effects of smoking on the woman, fetus and child and how to deliver brief cessation interventions to their clients.

PREGNETS includes representatives from the following agencies: Best Start Resource Centre, Ontario Tobacco Research Unit, Centre for Addiction and Mental Health, Clinical Tobacco Interventions, Motherisk (Hospital for Sick Children), Program Training and Consultation Centre, Smokers’ Helpline, St. Joseph’s Health Centre, and Toronto Public Health.
